Parents and caregivers can report an allegation of HIB by filling out and printing the Incident Form for Families below.
This can also be submitted to Mr. Regan or Mr. Mazzini by email.
If you would prefer to submit this confidentially, you can do so by mailing or dropping it off to their attention.
